How do I get copies of my medical records?
To request a copy of your medical records, call the Records Release office at 325-224-5264, or click on the link below to open and print the authorization form. Complete the authorization form and bring in or fax to our Records Release Office at 325-224-5913.
All blanks must be filled out. Contact the Records Release Office if you have questions or need assistance when completing the form.
